Cost-effectiveness analysis of palliative care: a systematic review of evidence and challenges

Since the foundations laid by Cicely Saunders, end-of-life care has undergone significant evolution. Although recognized as a fundamental human right in health, the available evidence on the cost-effectiveness of palliative care is limited. This research gap highlights the importance of deepening the study of its economic and social value. Savings have been demonstrated in health systems, exclusively at the hospital level, representing a cost-effective investment. However, the lack of research on palliative care at home creates a gap in research, highlighting the importance of further studying its economic and social value. The objective of this review is to provide information on palliative care that should be considered as an economic and social benefit in a holistic manner from the biological, psychological and social perspective. Methods: To better understand this dimension, a systematic review of cost-benefit analysis studies of palliative care was conducted, covering languages such as English, Portuguese, and Spanish. In order to ensure a comprehensive and representative search, specific search criteria were used in recognized databases such as PubMed, BVS, Cochrane, and Scopus. Out of a total of 2167 articles assessed, finally, 6 relevant studies were selected, emphasizing the rigor in selection to ensure the quality and relevance of the analyzed data. Results: The meticulous analysis of the included studies revealed various perspectives on cost-effectiveness, including comparisons between conventional treatments and palliative care, mixed-effects models, and evaluations of specific programs. This variety in approaches underscores the complexity and multifaceted nature of measuring the economic impact of palliative care. Conclusion: Despite the clearly documented benefits, greater standardization in the economic evaluation of palliative care is imperative to facilitate comparability and extrapolation of results to different healthcare contexts.
